In accordance with House Bill 4224 (89th Regular Session), effective September 1, 2025, The Oaks Life Center is required to provide the following information to all clients regarding their rights to access health care records and to file consumer complaints. 1. Requesting Your Health Care Records

You have the right to request a copy of your health care records from The Oaks Life Center. To request your records, you can follow these steps:

2. Contact the Licensing Council
